Pathology Lab Setup Cost in India: Equipment, Licences and Budget
Starting a pathology lab? This guide lists the machines a small lab needs and what each costs in India. It also covers licences, staff and running costs.
A small pathology lab in India costs about Rs 10 lakh to Rs 22 lakh to set up. A medium lab costs about Rs 27 lakh to Rs 57 lakh. These totals add up equipment, rooms, licences and software from a July 2026 cost study. The core machines are a hematology analyser (Rs 1.7 to 4 lakh) and a biochemistry analyser (Rs 40,000 to 6 lakh). Reagents and staff are monthly costs on top.

3-Part or 5-Part? Semi or Fully Automatic?
- 3-part hematology sorts white cells into three groups. It covers most clinic and small-lab CBCs at a lower price and reagent cost.
- 5-part hematology gives five white-cell groups. Hospital labs and labs serving specialists usually need it. See 3-part vs 5-part.
- Semi-automatic biochemistry needs a technician for each step; it suits up to a few dozen tests a day.
- Fully automatic biochemistry runs batches on its own; it pays off once daily volume grows.
Licences and Staff
- Registration under your state's clinical establishment rules.
- Biomedical waste authorisation from the state pollution control board.
- Signatory doctor: MBBS for a basic composite lab; MD or DNB (Pathology, Biochemistry, Microbiology or Lab Medicine) for medium and advanced labs.
- Technicians: at least 1 for a basic lab, 2 for medium and 4 for advanced, with DMLT, BSc MLT or MSc qualifications.
- NABL accreditation is optional but helps win corporate and insurance work.
The categories and staff numbers are from the Clinical Establishments (Central Government) Amendment Rules, 2018. Many states have their own rules, so check locally.
Running Costs and Earnings
The same breakdown puts monthly running costs at about:
- Rent and utilities: Rs 30,000 to Rs 80,000
- Staff salaries: Rs 50,000 to Rs 2 lakh
- Reagents and consumables: Rs 40,000 to Rs 1.5 lakh
- Equipment maintenance (AMC): Rs 10,000 to Rs 50,000
That is about Rs 1.3 lakh to Rs 4.8 lakh a month. On the income side, big private labs charged Rs 250 to Rs 449 for a CBC in September 2026. The CGHS rate is Rs 300. Ask each supplier for the reagent cost per test. Over five years, reagents often cost more than the machine.
How to Cut Lab Setup Cost
- Start with a 3-part hematology and a semi-automatic or compact automatic biochemistry analyser.
- Send rare or specialised tests to a reference lab until volume justifies the machine.
- Compare reagent cost per test and service response, not only the machine price.
- Choose analysers on an open or affordable reagent system with steady local supply.
Pulse Lab Analysers
| Model | Key details |
|---|---|
| Pulse ZT360 | Fully automatic 3-part hematology analyser; cyanide-free HGB; 10.4 inch touchscreen; stores over 100,000 records |
| Pulse ZT370 | Compact 3-part hematology analyser; 72 tests per hour; simple 2-reagent system |
| Pulse ZT Smart SAA | Compact clinical chemistry analyser for routine biochemistry |

Who can sign reports in a pathology lab in India?
Under the Clinical Establishments rules, a basic lab should have an MBBS doctor to sign reports. Medium and advanced labs need a doctor with an MD or DNB in a lab subject, such as pathology. Biopsy reports must be signed by a pathologist.
How much space does a small lab need?
A small lab can start in about 400 sq ft, growing past 2,000 sq ft as volume rises. It needs a sample collection area, a lit working area, a washing area, storage, a reporting and billing area and a toilet.
What does a CBC test cost patients in India?
Large private labs charged about Rs 250 to Rs 449 for a CBC in September 2026. The CGHS rate is Rs 300 at an accredited lab in a Tier I city.
Is NABL accreditation compulsory to start a lab?
No. NABL accreditation is optional for most labs, but corporate clients and insurers often ask for it. State registration under clinical establishment rules and biomedical waste authorisation are needed to operate.
